What Did the Study Find?
study evaluated serum REG3α, a Paneth-cell protein, across two established Crohn’s disease cohorts and a unique prediagnostic cohort.
Among 394 patients, higher REG3α independently predicted subsequent Crohn’s disease progression.
The association persisted after adjustment for CRP and endoscopic disease activity.
Higher REG3α was associated with approximately 2-fold greater progression risk across the clinical cohorts.
Remarkably, elevated REG3α before Crohn’s diagnosis predicted subsequent aggressive disease.
The strongest prediagnostic associations were with future stricturing/penetrating complications and surgery.
Predictive signals were detectable up to 10 years before diagnosis.
Why Does It Matter?
Current clinical and endoscopic assessment cannot reliably identify every patient destined to develop progressive Crohn’s disease. REG3α could potentially reveal biological risk not captured by conventional inflammatory markers.

REG3α is a promising risk-stratification biomarker, not yet a treatment-selection test. It could eventually help identify apparently mild patients who need earlier advanced therapy or tighter monitoring, but prospective validation and clinically actionable thresholds are required before REG3α should influence routine treatment decisions.
