Cirrhosis Care Should Become a Preventive Bundle—not a Series of Rescue Interventions: AJG | October 2026
What Did the Guideline Find?
The new ACG guideline translates preventive cirrhosis care into 18 GRADE-assessed recommendations and six key concepts spanning cirrhosis-specific complications, infection prevention, lifestyle interventions and non-hepatic comorbidities. Strong recommendations include six-monthly hepatocellular carcinoma surveillance, hepatitis A testing and vaccination when nonimmune, indicated influenza, COVID-19 and respiratory syncytial virus vaccination, smoking-cessation counselling, and carvedilol or endoscopic ligation for high-risk oesophageal varices.
Conditional recommendations include lactulose for minimal hepatic encephalopathy, adjunctive rifaximin following hospitalisation, short-course antibiotic prophylaxis during upper-GI bleeding, structured treatment for alcohol-use disorder after discharge, dietitian-led nutritional care, supervised exercise and bisphosphonates for osteoporosis. Importantly, most recommendations rest on low-certainty evidence despite their clinical plausibility and established preventive value. gi.org
Why Does It Matter?
The guideline supports using a structured checklist at every cirrhosis visit rather than waiting for decompensation. Vaccination, cancer surveillance, bone health, nutrition, alcohol treatment and cognitive assessment should be treated as core hepatology—not ancillary primary care.
GastroAGI Takeaway
Build a cirrhosis prevention bundle into every clinic visit: surveillance, vaccination, cognition, nutrition, lifestyle and bone health are disease-modifying care.
