Fatty Pancreas Predicts Diabetes, Kidney Disease, and Cardiovascular Risk: UEG Journal | July 2026

Introduction:

Fatty pancreas is increasingly recognized as an ectopic fat depot with metabolic consequences, but its clinical significance has been difficult to define because previous studies used inconsistent thresholds. This large UK Biobank study applied the 2026 international MRI-PDFF consensus criteria to evaluate whether pancreatic fat predicts type 2 diabetes, chronic kidney disease (CKD), and major cardiovascular events.

Why was this study needed?

Fatty pancreas has been linked to insulin resistance and β-cell dysfunction.

Standardized MRI thresholds for pancreatic fat were only recently established.

Its relationship with CKD and cardiovascular outcomes remains uncertain.

Large prospective validation was needed before pancreatic fat could be used in cardiometabolic risk assessment.

Results:

Among 19,255 participants, both mild and moderate-to-severe fatty pancreas were associated with higher risk of incident type 2 diabetes, with the strongest effect at PDFF ≥16%.

Moderate-to-severe fatty pancreas also independently predicted incident CKD.

Associations with major cardiovascular events were present but more modest.

Risk increased progressively with pancreatic fat, particularly for diabetes, even after adjustment for obesity and visceral adiposity.

Clinical Impact:

These findings position fatty pancreas within the broader cardiovascular–kidney–metabolic continuum. MRI-derived pancreatic PDFF may provide additional metabolic risk information beyond BMI and visceral fat, especially in patients already undergoing abdominal MRI. However, pancreatic fat should complement—not replace—standard cardiometabolic risk assessment.

Bottom Line:

Fatty pancreas is not merely an imaging finding. Consensus-defined pancreatic fat, especially PDFF ≥16%, is independently associated with future type 2 diabetes and CKD, with a smaller signal for cardiovascular events. Pancreatic fat may emerge as a useful marker of systemic metabolic risk.
