Introduction:

The relationship between alcohol consumption and inflammatory bowel disease (IBD) has remained controversial. This large prospective study demonstrates that the effect of alcohol depends on an individual's genetic capacity to metabolize acetaldehyde, highlighting a new paradigm for precision nutrition in IBD.

  • Acetaldehyde promoted intestinal inflammation, while acetate showed protective effects, and experimental studies confirmed that altering aldehyde dehydrogenase (ALDH) activity directly influenced colitis severity.

Clinical Impact:

This study suggests that one-size-fits-all recommendations regarding alcohol consumption are inappropriate for IBD prevention. Instead, genetic variation in alcohol-metabolizing enzymes (ADH/ALDH) may help identify individuals who are more susceptible to alcohol-induced intestinal inflammation and could form the basis for future precision nutrition strategies.
