Liquid Nitrogen Spray Cryotherapy Offers Repeatable Palliation in Advanced Oesophagal Cancer: GIE | August 2026
Introduction:
Dysphagia from advanced unresectable oesophagal cancer can severely impair nutrition and quality of life. Conventional palliation often relies on stenting or feeding-tube placement, both of which carry important limitations. This prospective cohort study evaluated whether serial liquid nitrogen spray cryotherapy (LNSC) can provide durable symptom relief while delaying or avoiding these interventions.
Why was this study needed?
Malignant dysphagia remains difficult to palliate effectively.
Esophageal stents can relieve obstruction but may cause pain, migration, reflux, or other complications.
Cryotherapy offers a non-contact ablative approach that can be repeated.
Prospective data on quality of life and longer-term need for stenting or feeding support were limited.
Results:
Among 49 patients, repeated LNSC produced modest but sustained improvement in general and esophageal cancer–specific quality of life.
Most patients were able to avoid immediate feeding-tube or stent placement; about 40% eventually required one, at a mean of roughly 8 months.
Conventional ordinal dysphagia scores did not significantly improve, highlighting a disconnect between symptom scoring and broader quality-of-life benefit.
Treatment-related serious adverse events were uncommon, with only two possibly related events.
Clinical Impact:
LNSC appears to be a useful repeatable palliative option for selected patients with obstructive oesophagal cancer, particularly when the goal is to preserve oral intake and delay stenting or feeding-tube dependence. It may fit well within a multimodal palliative strategy rather than replacing established approaches outright.
Bottom Line:
Serial liquid nitrogen spray cryotherapy can provide durable quality-of-life benefit and delay the need for oesophagal stenting or feeding-tube placement in many patients with advanced obstructive esophageal cancer, with a favorable safety profile.
