Pruritus in Primary Sclerosing Cholangitis: Hepatology | August 2026

Introduction:

Pruritus is one of the most troublesome symptoms experienced by patients with primary sclerosing cholangitis (PSC), yet its natural history has been poorly understood. As disease-modifying therapies remain unavailable, improving symptom control has become a major priority for patients and clinicians. This prospective multicenter study evaluated the prevalence, severity, persistence, and clinical significance of pruritus in PSC, providing valuable data to guide both clinical care and future therapeutic trials.

Results:

The study prospectively followed 220 patients with PSC across multiple centers.

More than half of patients experienced pruritus, while approximately one in four reported moderate-to-severe itching.

Patients with cirrhosis, advanced liver stiffness, or previous episodes of ascending cholangitis experienced significantly more severe pruritus.

Pruritus severity correlated with markers of cholestatic liver disease, including bilirubin, alkaline phosphatase (ALP), ALT, and AST, indicating that worsening itch reflects greater disease severity.

Increasing pruritus was associated with marked deterioration in health-related quality of life, affecting both disease-specific and general well-being.

Among patients with moderate-to-severe pruritus, over 60% experienced persistent symptoms throughout the 48-week follow-up, demonstrating that itching is often a chronic rather than transient problem.

Nearly half of patients experienced spontaneous improvement in itch intensity, even without initiating new antipruritic therapy, highlighting the natural variability of symptoms and the importance of accounting for placebo effects in future clinical trials.

Clinical Impact:

This study establishes pruritus as a common, persistent, and clinically meaningful manifestation of PSC, closely linked to disease severity and reduced quality of life. The findings emphasize that symptom assessment should become a routine component of PSC management rather than being viewed as a secondary concern. Importantly, the observed natural fluctuation in pruritus provides critical information for the design of future clinical trials, particularly regarding treatment duration, endpoint selection, and expected placebo responses.

Bottom Line:

Pruritus affects more than half of patients with primary sclerosing cholangitis, with one in four experiencing moderate-to-severe symptoms that often persist over time. Greater itch severity is associated with more advanced liver disease and poorer quality of life, making pruritus an important therapeutic target and a key patient-centered outcome in future PSC clinical trials.
